I understand that if I experience any adverse reaction, unexpected symptom, or medical concern, I should discontinue use and seek appropriate medical care.

I certify that I am at least 18 years of age and legally capable of entering into this agreement.

To the fullest extent permitted by law, I voluntarily assume responsibility for my decisions regarding the use of products and services offered by NUE Enterprises LLC.
